African American AIDS Task Force (AAATF):  A community-based non-profit agency founded in 1995 by African American men and women living with HIV/AIDS who were seeking a means to address the disproportionate way in which HIV/AIDS was affecting African American people living in Minnesota.  AAATF provides culturally-appropriate HIV/AIDS prevention, education and services to African Americans and others of African descent who are living with or at risk of HIV infection.

The Aliveness Project:  A community drop-in center encouraging self-empowerment and providing direct services to over 1,400 individuals living with HIV/AIDS each year. Services include an on-site hot meals program, food shelf, complimentary therapy program, HIV prevention, care advocacy and holiday basket program. Each year, over 1,000 individuals volunteer over 30,000 hours. 

Clare Housing:  A non-profit providing quality care and housing to people disabled by HIV, allowing them to maintain their dignity at any stage of the illness. With programs supporting each individual's personal journey with HIV, Clare Housing provides a range of life skills services, mental health and chemical dependency services, as well as on-going case management and support groups to ensure increased self-esteem and independence of each client.

Between four foster homes (Agape Home, Agape Dos, Damiano, and Grace House II) Clare Apartments, and Clare Midtown, the organization operates half of the housing designated for people living with HIV/AIDS in Minnesota, establishing them as the largest provider of HIV housing and services in the state.  

Hope House of St. Croix Valley:  An adult foster care facility for people living with HIV/AIDS, provinding a non-judgmental home with compassionate care,respecting the dignity of people living with HIV/AIDS. Hope House has served the Washington County and the Greater Twin Cites area since December 1993. 

Minnesota AIDS Project (MAP):  Serving the community as a trusted resource for information, education, and support since 1983. The Minnesota AIDS Project works state wide to prevent the spread of HIV and reduce the challenge of living with HIV through programs, partnerships and initiatives to engage every member of our community to take responsibility to stop HIV. 

One Heartland:  One Heartland is a national non-profit organization dedicated to greatly improving the lives of children, youth and their families facing HIV/AIDS and other life challenges worldwide. Founded in 1993 as a summer camp for children affected by HIV/AIDS, One Heartland (formerly Camp Heartland) provides a variety of year-round programs for children living with HIV/AIDS; reaches young people at risk for HIV infection with AIDS awareness initiatives; and serves additional marginalized populations through its Heartland Partners program. One Heartland provides children and young adults with opportunities to have fun, develop new skills, make friends, and speak openly about their life experiences at its facility in northern Minnesota.

One Heartland’s Camp Heartland Program is the largest camping and care program for children who experience HIV/AIDS. Over 75 percent of Camp Heartland’s participants live in poverty. One Heartland’s programs and services are provided free of charge to all participants from over 40 states across the country.

Park House:  An organization that believes everyone needs a community to find safety, strength, and support with a Day Health / Mental Health Day Treatment Program for persons living with HIV/AIDS. Services include: nursing support and medication regime management; nutrition, psychosocial counseling and psycho-educational and therapeutic group activities; creative (art, writing, movement) and complementary therapies (massage and shiatsu); and individual and group socialization activities. Park Houses opened its doors on World AIDS Day, December 1, 1996. It is the first and remains the only Day Health / Mental Health Center for adults living with HIV/AIDS in the upper Midwest.

Rural AIDS Action Network (RAAN):  A non-profit, community-based organization with a mission to organize, develop and sustain caring communities of professionals and volunteers serving and supporting persons living with, affected by, or at risk for HIV/AIDS in the rural Midwest. Throughout Greater Minnesota, RAAN provides case management, HIV testing, prevention education, professional training, transportation assistance, emotional support and assistance with medical bills. It's the only state-wide organization dedicated to ensuring HIV/AIDS services to Minnesota's rural communities at the local level.
